Generally "coming undone" means to be pulled apart or loosened, like at a seam. I'd think in this case that it's referring to mental degradation, going crazy or not being able to hold yourself together. It's like a mental snap.

I think it's a kinda negative word.. Like.. Being upset/angry/sad or breaking down or something? Sort of like losing control? It can mean a lot of things (see the link, it's really good :) Best of luck.
