if you use it in that was, it usually means that you attempted to do something and sucked at it, usually in a funny way (at least to others its funny)
like, at lunch there are awkward pauses across the whole school and my friend tries to start them. everyone usually just goes silent for a few seconds, then starts laughing. my friend trys to start this by yelling and getting attention, then falling silent. if it does'nt work, we just say "fail".

Well they could be calling you a failure for saying whatever it was that you said. Generally it means "You've failed"
e.g. skateboarder ollies onto a rail but slips and hits his groin. fail
cat tries to jump up on the counter but slides across to the other side and falls off. fail
computer gives the blue screen of death. fail.

Like for example: I am a very clumsy person so when someone sees me they say "Fail" Like i failed or i just embarassed myself! So ppl say "Fail" if you do something rong like failing.
