Ground Control is the name for the base which is monitoring and coordinating the mission of the space launch and mission (usually NASA). Major Tom is the person in charge out in space on the mission. Ground Control is speaking to Major Tom when they say that.

When someone says that to another person as a form of slang, then they are implying the other person is off in their own little world somewhere inside their head, spacing out, and they are calling out to them to get them situated, to get them grounded in reality. It's a way of saying "Are you there? Are you hearing me? Am I getting through?".

Ground Control is the complement of scientists/technicians on the Earth that are controlling/communicating with astronauts in space. Major Tom is an astronaut.

Ground Control is the people back at the space station and they are communicating with major Tom who is the astronaut.

i believe major tom is supposedly on drugs or has a drug problem, in a later song, 'ashes to ashes', bowie directly refers to major tom as being a junkie. with that in mind, i see "ground to control to major tom" basically just like a friend trying to get through to someone if theyre 'spaced' out. may not make any sense at all, but thats life.

also, ive seen before someone suggesting maybe major tom is just a guise for a drug itself, but who knows. its completely open to your own interpretation as far as i see

I know that this is an old question but reading the answers prompted me to interject. It has nothing to do with space, NASA or even astronauts. It has to do with mind control. Do what the leader tells you and everything will be fine, but in the end it's not. You have been fooled. Bowie was never literal.

Its about an astronaut in space and all the things he sees and feels.
